The Four-spotted Fungus Moth has a wingspan of 1 3/8 inches. Pale reddish-brown wings have dusky scales. All lines are indistinct except for the fragmented black median bands. The reniform spot and subapical patch are black. The caterpillar is brown, with bumps and sparse, short hairs. It resembles other fungus moth caterpillars, so identification should be based on the adults.

The Four-spotted Fungus Moth was photographed coming to lights at Fontenelle Forest Nature Center during the 2015 Bioblitz on May 30th. It was also photographed at moth lights on June 8, 2017 in Fontenelle Forest. It is considered to be common in its range.

The larval hosts are fungus (usually bracket fungi), mushrooms and fungal mats.
